Game ini mengambil latar belakang waktu tahun 2281 yang berarti 4 tahun setelah kejadian dalam Fallout 3 atau 204 tahun dari Great War pada tahun 2077 . Tempat yang diambil oleh Fallout : New Vegas adalah di Mojave Wasteland yang memiliki ukuran yang sama seperti Capital Wasteland di Fallout 3. Dan disebutkan bahwa letak dari Mojave Wasteland adalah antara Nevada dan Arizona. Tidak seperti kota-kota lain yang pernah ada di seri Fallout lainnya, kota Las Vegas di Fallout : New Vegas tidak mendapat serangan langsung senjata nuklir sehingga masih dapat ditemukan bangunan yang berdiri tegak dan hanya terjadi mutasi yang minimal pada organisme di kota itu.
Di dalam game ini, player sebagai seorang kurir yang bertugas membawa sebuah paket berisi Platinum Poker Chip untuk diantarkan ke New Vegas. Namun, di tengah jalan Anda akan dirampok oleh seorang pemimpin kasino dari New Vegas bernama Benny dan ia akan menembak palayer sebagai sang kurir. Lalu, seorang robot datang menemukan tubuh Player dan membawa tubuh sang kurir kepada Dokter Mitchell di Goodsprings. Di sini, Player sebagai pemain akan masuk dalam mode kustomisasi karakter antara lain skill, nama, umur, gender, dan penampilan. Pemain akan memulai petualangan untuk membalas serangan yang dialaminya dengan petualangannya di Mojave Wasteland dan merebut kembali barang yang dirampok oleh Benny. Dalam petualangan, pemain akan menemukan banyak kejadian dalam pengejarannya terhadap Benny. Namun setelah mendapatkan Benny dan chip yang dicurinya, sang kurir akan mendapati dirinya berada di tengah-tengah persaingan antara dua faksi,
Untuk yang sudah akrab dengan Fallout 3 Mimin harap kalian tidak akan sulit beradaptasi dengan game ini. Fallout New Vegas memiliki gameplay yang sama dengan Fallout 3. Mekanisme gameplay masih mengikutsertakan opsi pengembangan karakter dengan atribut S.P.E.C.I.A.L
